Network #15 patients - image of family

Topics in this section introduce subjects--including recommendations--that may interest people with end-stage renal disease. Much of the information is targeted toward patients who live within the intermountain (Network #15) region: Arizona, Colorado, Nevada, New Mexico, Utah, and Wyoming.

Network #15 encourages patients and their families to visit our "Links" page for more, general information on ESRD and related health issues. Thenew Kidney Schoolwebsite is especially insructive. In case of emergencies such as natural disasters we encourage patients to go to our "Disaster Resources" page in addition to reviewing the CMS Publication #10150 Preparing for Emergencies (pdf 37 pgs).

Most importantly, we recommend that patients engage their care team (physicians, nurses, social workers, dietitians, and technicians) in the decision-making process, to steer a balanced and optimal approach to treatment.

Network #15 can be contacted at the following toll-free phone numbers, to be used only by patients:

1-888-777-0105 and 1-800-783-8818.

Network #15 does not handle issues dealing with money, payment of bills, or state or federal licensure or certification issues. Please visit the Centers for Medicare and Medicaid Services Web site for more information about these kinds of issues.

Patient Leadership Committee

The Patient Leadership Committee (PLC) is a group of kidney patients who:
1. Help ESRD Network #15 respond to patient needs.
2. Help educate patients about becoming more healthy.
3. Represent and advocate for patients within the ESRD Network #15.

If you might like to work on this committee, please print out and complete this form (pdf) and send it to the following address:
