Notify the Network of your intention to open a new unit

• Fill out and print the Initial Facility Info Form (pdf) then Fax to Network #15 at 303-860-8392.
  

Review information from the three main departments in Network #15:

• Quality Improvement Dept       • Patient Services Dept       • Data Dept

• WebEx 1 - Network #15 Overview (20 minutes) - An overview of the ESRD Network organization, Network responsibilities, and aspects of Quality Improvement and Patient Services. Appropriate for new Data Contacts, Medical Directors, Clinical Managers, Administrators, and Social Workers.

• WebEx 2 - Understanding the Patient Activity Report (PAR) - (27 minutes) - A detailed look at the ins and outs of patient-event reporting. Appropriate for the Data Contact or anyone who completes this CMS-required monthly report.

• WebEx 3 - 2728 and 2746 Forms & Compliance (28 minutes) - A tutorial on the CMS Medical-Evidence Form (2728) and the Death Notification Form (2746). Highlights fields that are frequently confusing and emphasizes the importance of accuracy and timeliness. Appropriate for Social Workers and Data Contacts who work on these two forms.
