Timeline - The timeline and requirements for users of CROWNWeb will likely undergo addtional changes. Network #15 will continue to post information as it becomes available from CMS.
- February 1, 2009 - Phase 1 began with a total of eight providers/facilities selected from four different Networks.
- Phase II began August 3, 2009 and included 10 dialysis units from each of the 18 Networks. The ten units from Network #15 are all in the Denver Metro area and are listed below.
- Phase II Expansion - Rolled out early May 2011 - Ended November 30, 2011
- Phase III - Began January 17, 2012. Just over 20 facilties from Network #15 will be participating in this Phase.
- Full National Release - CMS has moved its target date to March of 2012 for the national release.
- QIMS Registration - Phase III Facilities - CMS CROWN Memo dated Dec 20, 2011 announced that Phase III facilties could begin registering in QIMS Dec 16, 2011.
- QIMS Registration - All Users - Beginning January 30, 2012
Overview
The new Conditions for Coverage, published April 15, 2008, required the electronic submission of administrative and clinical data by all Medicare-approved dialysis facilities. To support this mandate, CMS developed a Web-based software application known as CROWNWeb. To use CROWNWeb facility staff must complete a multi-step registration process.

For access to CROWNWeb releases, ESRD Network and facility staff must register for a Quality Information Management System (QIMS) account by completing online Security Awareness Training, and both parts of the QIMS Account Form. This process has proved to be quite complicated so we recommend people review the QIMS Online Help PRIOR to completing the forms below:
- Part A - Used to document the identity information required for all users to have access to CROWNWeb. Can only be completed online. Applicant needs to hit "Print Screen" to print a paper copy of Part A. This printout needs to be signed, and applicant's identity needs to be verified in person by an SO, or notarized if no local SO is available. If a local End-User Manager is available they must also sign Part A, or If no local End-User Manager is available, the Help Desk will act as the EUM.
- Part B (PDF) - Defines a user's role and scope in CROWNWeb. May be completed electronically and then printed, or downloaded and then completed by hand. Needs to be given to the End User Manager for signature and configuration in CROWNWeb.

Security Awareness Training
In addition to completing a QIMS application online, CROWNWeb users will also be required to complete an online Security Awareness Training and print a certificate verifying this. If you are the first Security Official (SO) you will need to send a copy of your certificate to the helpdesk along with Part A and Part B of the QIMS account form.
Here are some general tips to be aware of before you begin the online Security Awareness training:
- The training will take you about one hour and 30 minutes if you do not fast forward through anything.
- You can pause the training and then finish at a later time if needed –but you must be using the same computer.
- Be sure that you are using a computer that has access to a printer as you will need to print a certificate of completion to prove that you have finished the training.
To complete the online Security Awareness Training:
- Follow this link to launch the Security Awareness Training module: http://iase.disa.mil/eta/iss_icv5/launchpage.htm.
- You should allow Active X to show pop-ups for the site.
- You will want to turn up your speakers so you can hear the presentation.
- You will not be scored on whether you answer questions correctly, just on whether you complete the entire training.
- At the end of the module, you will be given an opportunity to review the security tips and print a summary – this is optional.
- Do NOT exit the Security Tips page until after you have hit the “PRINT CERTIFICATE” button.
- Type your name in the online display for the Certificate of Completion, and print a copy. (Note: Certificates with a handwritten name will not be accepted.)
- Sign the Certificate of Completion and keep your copy in a safe place. You will need to provide a copy of this as part of the new registration process for CWeb.
Receiving User ID and Password
When a new account is created in QIMS, the system will generate e-mail notifications to account holders providing the User ID and temporary Password (in separate messages).
Security Officials will receive TWO user IDs and TWO passwords (in four seperate emails), because SOs will have one regular account for accessing CROWNWeb (through QIMS), and one SO account for accessing Administration of other Users (through QIMS.) People who register to have scope over multiple facilities will receive a separate User ID and Password for each of those facilities.
The new user will be asked to log in to the QIMS system to create a new password and answer six authentication questions. Users will then be ready to access QIMS and/or the CROWNWeb system.
