CROWNWeb
Overview
The new Conditions for Coverage, published April 15, 2008, require the electronic submission of administrative and clinical data by all Medicare-approved dialysis facilities. To support this mandate, CMS has developed a Web-based software application known as CROWNWeb. To use CROWNWeb facility staff must complete a multi-step registration process.
Registering to Use CROWNWeb Via QIPS Forms
Personnel must register for CROWNWeb via the QIPS Account Form. Two basic categories of users exist: Regular Users and Security Administrators (SAs). Every dialysis unit must register at least one person as an SA (and should probably have two); the number of additional, "regular" users is up to each unit, but we strongly recommend each facility signs up multiple users. People you might consider signing up as “regulars” would be staff with experience filling out 2728 and 2746 forms, the person who completes the PAR, and people familiar with submitting clinical information like Fistula First forms and Clinical Performance Measures.
Please read the instructions below BEFORE filling out the QIPS form. CMS is VERY particular about how the QIPS forms are filled out. If there are ANY errors or omissions CMS will REJECT the QIPS form and require you to send a new one.

QualityNet Identity Provisioning System (QIPS)
QIPS website - Maintains user account information including passwords and ID's needed to access CROWNWeb. Security Administrators log in to QIPS in order to create and update regular user accounts; Regular Users log in to QIPS to update their own CROWNWeb password and to answer authentication questions that may be used if a password is forgotten.
QIPS Password User Guide - This guide created by CMS covers first time logging into QIPS, changing from temporary to permanent password, answering authentication questions, and modifying personal information.
Note: If you attempt to login to QIPS using the wrong password your account will be locked out after 3 attempts. You should call Network #15 at 303-860-1515 and ask for Jesse if your password needs to be reset.

Timeline
- February 1, 2009 - Phase 1 began with a total of eight providers/facilities selected from four different Networks.
- Phase 2 began August 3, 2009 and includes 10 dialysis units from each of the 18 Networks. The ten units from Network #15 are all in the Denver Metro area and are listed below.
- Phase 3 will likely encompass all Medicare dialysis facilities. CMS has not given any suggestion of a start date but has indicated that they will attempt to provide some amount of lead time for facilities to prepare. As Network #15 learns more about the CROWNWeb timeline we will keep you updated.

Beginning August 3, 2009 each of the 18 ESRD Networks have ten facilities participating in Phase II of CROWNWeb. The following are the facilities from Network #15:

Online Helpdesk Customer Portal - This site contains a "knowledgebase" of frequently asked questions, but requires registration to enter. To register input your CROWNWeb User ID and email associated with your CWeb account. You will then need to create a NEW password that is at least 8 characters long, contains both upper and lower-case letters, and has both a number and a special character ( @,#,!, etc.)
