Dear New ESRD Network #15 Dialysis or Transplant Facility Staff,
Welcome to our Network! As the staff members of the Patient Services Department, we would like to provide you with some information that has been previously disseminated to our facilities. Please download and review the resources in the box to the right addressing patient psychosocial, grievance, and disaster preparedness issues. You will need to complete and return the Spanish Readers Survey. Additionally, you will be receiving in the mail laminated Network #15 Grievance Posters in English and Spanish which will need to be posted at your clinic.
In addition to the documents at right you can link to more Patient Services resources throughout our website. Provider resources include: Decreasing Dialysis Patient-Provider Conflict (DPC) Toolkit, vocational rehabilitation, emergency preparedness, patient quality of life, and advance care planning. There are also patient resources in English and Spanish that cover pertinent topics such as patient rights and the Network grievance protocol, treatment modalities, vascular access and dialysis adequacy, and links to Dialysis Facility Compare, as well as links to other CKD-related web sites.
One of our key functions is to provide technical assistance to facilities with patient-provider conflicts, where efforts at resolution may have come to an impasse, or include a pending consequence of involuntary discharge for the patient. Please call us for our perspectives and suggestions--we may be able to help you adhere to the CMS Conditions for Coverage while achieving a good outcome for the patient.
